ANNUAL EMOTIONAL HEALTH SURVEYS


IT'S TIME TO CHANGE HOW WE VIEW A CHILD'S GROWTH
EMOTIONAL GROWTH MATTERS TOO!

It's natural to measure your child's height & weight. Measuring other ways they grow is important too! Emotional health issues can affect how a child succeeds in school, how they relate to family members, & their ability to make friends. Completing an Emotional Health Survey is a simple, effective way to look at your child's emotional development. If issues are identified early, intervention can be effective.


The Child & Family Clinic Plus Emotional Health Survey is Free, Confidential & Voluntary. It is offered by Family Services of Chemung County, Inc. to all students in Chemung County. Results are only shared with the parents or guardians. If a student's survey indicates a need for emotional health services, we can discuss resources available to you in the community, such as talk therapy.


PreK through 5th grade - to complete a consent form and survey click here.

Sixth Grade through 12th grade - The survey can be completed by either your student or you. Click here for consent form and survey.
